Having a Health Care Proxy and completing a Living Will are vital steps to ensure that healthcare providers and loved ones know the care you choose to receive or not receive in a medical crisis if you’re unable to speak for yourself. Having your preferences honored and respected gives peace of mind to everyone involved.
Led by social worker Barbara Sarah and Carolyn Van Ness, Nurse Practitioner and Certified Advance Care Planning Facilitator, this free workshop is designed to answer confusing questions and assist participants in completing their Advance Directives. Participants will receive a complimentary New York State Advance Directive form, which Sarah and Van Ness will guide you through filling out and getting all your questions answered.
